 MUST HAVE CERTIFIED BIRTH CERTIFICATE THAT LISTS PARENT(S) NAME(S) & HAS STATE SEAL.

NOTE:  Adult applicants renew passports by mail and do not need to book an appointment (use the DS-82 Application).
